1. Scope
This Privacy Policy explains what information dialysiscenters.org (the "Site") collects about visitors, and what we do with it. It applies to the Site and its subdomains.
This policy is about information we collect from you. It does not cover the facility information published on the Site, which comes from public data released by the Centers for Medicare & Medicaid Services and is not personal information about visitors.
Washington residents and anyone whose information is collected in Washington should also read our Consumer Health Data Notice, which covers rights specific to that state's My Health My Data Act.
Our Terms of Service govern your use of the Site.
2. Information We Collect
Searches. When you search for facilities, we receive what you type and use it to find facilities near that location. We do not ask for or use your device's location.
Contact form. If you write to us, we receive your name, email address, and message.
Server logs. Our hosting provider records standard technical information with each request, including IP address, browser type, the page requested, and the time of the request.
Analytics. We run our own analytics software to count page views and understand which pages are used. It records the page visited, referring site, approximate region, browser, and device type. It is configured to honor Do Not Track browser signals and to exclude search terms.
3. How We Use Information
We use what you type in a search to find facilities near that location and return results to you.
We use contact form submissions to respond to you and to review reported problems with the Site.
We use server logs and analytics to operate the Site, diagnose errors, protect against abuse, and understand which pages are used.
4. Service Providers
We use service providers to host the Site, serve map images, convert typed locations into coordinates, and measure usage. Each receives only what it needs to perform its function.
Our Consumer Health Data Notice lists these providers by name.
These providers act on our behalf. They are not permitted to use what they receive for their own purposes.
5. Sharing and Sale
We do not sell personal information, and we have never sold personal information.
We do not share personal information for cross-context behavioral advertising.
Apart from the service providers listed in Section 4, we disclose personal information only when we believe in good faith that disclosure is required by law, legal process, or a government request, or is necessary to protect our rights, the Site's security, or the safety of any person.
6. Cookies and Similar Technologies
The Site does not set cookies, and our analytics software does not set cookies or use any similar identifier to recognize you across visits or across other websites. Your browser stores a single setting locally to remember that you have dismissed our terms notice; it is not an identifier and is not sent to us.
Because we do not use cookies, we do not ask for cookie consent.
7. Retention
We keep contact form submissions for as long as needed to respond and to maintain a record of reported problems with the Site.
Server logs are retained by our hosting provider for a limited period and are used to operate the Site, diagnose errors, and protect against abuse.
8. Your Choices
Access and deletion. You may ask us what personal information we hold about you and ask us to delete it. Write to support@dialysiscenters.org. We will verify your request before acting on it, and we will respond as required by applicable law.
California residents. California law gives residents the right to know what personal information a business collects, to request deletion, to correct inaccurate information, and not to be discriminated against for exercising those rights. You may exercise these rights at the address above.
Washington residents. Washington's My Health My Data Act provides additional rights regarding consumer health data. Our Consumer Health Data Notice describes those rights and how to exercise them.
Browser settings. Most browsers let you block or clear site data and send a Do Not Track signal. These controls are provided by your browser, not by us.
9. Children
The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13.
If you believe a child under 13 has sent us personal information, write to support@dialysiscenters.org.
10. Security
We use reasonable measures to protect the information we hold. No method of transmission or storage is completely secure, and we cannot guarantee the security of information you send us.
11. Changes
We may update this Privacy Policy. A revised version takes effect when we post it, and we will update the "Last updated" date above. If we make a material change in how we handle personal information, we will say so on the Site.
