Please include the facility's name or a link to its page on our site, along with any details you'd like to share. That helps us find the right center quickly.

When to reach out somewhere else

We can't answer medical questions or contact a facility for you. For questions about your treatment, please talk with your nephrologist or care team. For appointments or anything about a specific center, contact that facility directly.

To share a concern about the care at a dialysis facility, you can contact your regional ESRD Network or your state's health department. For questions about Medicare coverage, call 1-800-MEDICARE (1-800-633-4227).
