Consumer Health Data Notice

1. Scope and Why This Notice Exists
Washington's My Health My Data Act requires a separate notice about consumer health data. This notice covers dialysiscenters.org (the "Site") and explains what we collect, why, who receives it, and what rights you have.
It applies to Washington residents and to anyone whose information is collected while they are in Washington.
This notice is about information we collect from you. The facility information published on the Site comes from public data released by the Centers for Medicare & Medicaid Services. That data is about healthcare institutions, not about visitors, and this notice does not cover it.
Our Privacy Policy covers information practices generally. Our Terms of Service govern your use of the Site.
2. What We Collect
What you type in a search. When you search, we receive the location you enter. We do not ask for or use your device's location, and we do not collect precise location data.
What you send us. If you write to us through the contact form, we receive your name, email address, and message.
Technical information. Our hosting provider records standard technical information with each request, including IP address, browser type, the page requested, and the time of the request.
Usage measurement. We measure which pages are used. This records the page visited, referring site, approximate region, browser, and device type. Search terms are excluded.
We do not have accounts, and we do not ask you for health information about yourself.
3. Why We Collect It
We collect the location you type in order to find facilities near it and return results to you. That is the service you asked for, and we use what you type only to provide it.
We use contact form submissions to respond to you and to review reported problems with the Site.
We use technical information and usage measurement to operate the Site, diagnose errors, and protect against abuse.
We do not use any of this for advertising, and we do not combine it with information from other sources to build a profile of you.
4. Who Receives It
We use the following service providers to operate the Site. Each receives only what it needs to perform its function, and none is permitted to use what it receives for its own purposes.

We measure usage with analytics software running on infrastructure we control.
Apart from these providers, we disclose information only when we believe in good faith that disclosure is required by law, legal process, or a government request, or is necessary to protect our rights, the Site's security, or the safety of any person.
5. We Do Not Sell Consumer Health Data
We do not sell consumer health data, and we have never sold it.
Washington law requires a signed valid authorization before any sale of consumer health data. Because we do not sell it, we do not seek such an authorization.
6. Your Rights
Washington's My Health My Data Act gives you the right to:
- confirm whether we collect, share, or sell your consumer health data, and access that data;
- obtain a list of the third parties with whom we have shared or sold it, along with contact information for each;
- withdraw consent to our collection and sharing of it; and
- have it deleted.
If you ask us to delete your consumer health data, we will delete it from our records and direct our service providers to delete it from theirs.
We will not deny you access to the Site, charge you a different price, or provide you a different level of service because you exercised any of these rights.
7. How to Exercise Your Rights
Write to support@dialysiscenters.org. Tell us which right you are exercising.
We will verify your request before acting on it. We may need to ask you for information to do that, and we will use what you give us only to verify the request.
We will respond within 45 days of receiving your request. If we need more time, we will tell you within that period and may extend it by a further 45 days.
If we decline your request, we will tell you why and how to appeal. If we deny your appeal, we will give you a way to contact the Washington Attorney General.
